Program Overview:

NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs.


Job Title: Community Enrollment Specialist


Job Summary:

The Community Enrollment Specialist (CES) provides written, face to face and telephonic enrollment and orientation services for interested Health Home referred members. The Community Enrollment Specialist will work closely with the Outreach Support Specialists to assess and enroll as many interested clients as possible. The CES will provide hospital follow-up services to assigned members.


  • Provide intake and screening for all interested Health Home referred members 
  • Maintain regular follow up on cases as needed
  • Provide hospital follow-up to NADAP assigned members as per departmental policy
  • Obtain all appropriate consent and enrollment forms for each client 
  • Ensure that each enrolled member is transitioned to a Care Coordinator via a warm hand-off
  • Maintain accurate case records in HHDB and assigned EHR systems
  • Provide training support and mentoring for new staff members
  • Utilize telephonic and electronic outreach through databases to obtain valid contact, insurance, demographic, and diagnostic information as needed to locate eligible participants and coordinate enrollment, follow up, and status updates for each client
  • Uphold all HIPAA regulations and confidentiality protocols to record each interaction with every client served 
  • Performs other duties as assigned

This position requires the candidate to be on the field about 75% of the time and take mass transit to travel from one site location to another.


  • High School Diploma required
  • Minimum (1) year of experience in serving populations living with chronic medical and/or mental health conditions
  • Bilingual Spanish, Russian, or Chinese a plus
  • Excellent interpersonal, organizational, writing and computer skills
  • Must be able to travel via mass transit within New York City 75% of the time
