Program Overview

NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs. 

 

Job Title: Outreach Support Specialist – New York, NY

                                                                       

Responsibilities

The Outreach Support Specialist (OSS) provides written and telephonic outreach and orientation services for assigned Health Home members.The OSS works closely with Outreach Supervisors staff and the Outreach team to organize client enrollments.

  • Provide progressive and meaningful telephonic and written outreach and orientation services for each assigned member in order to facilitate
  • Health Home screening, orientation, enrollment, engagement and retention.
  • Utilize telephonic and electronic outreach, internet databases, EHR(s), NADAP databases, coordination with clients, known providers, MCOs and MAPP searches to obtain demographic and treatment information as needed to locate eligible participants and coordinate enrollment.
  • Enter contact notes documenting all outreach efforts within 2 business days of attempted/completed service delivery. Ensure that notes are entered into two online systems which are mirrored and up to date.
  • Uphold all HIPAA regulations and confidentiality protocols to record each interaction with every client served (including demographic and diagnostic information). Report possible infractions to direct supervisor immediately.
  • Maintain accurate case records in online systems (e.g. process discharges in a timely fashion) as outlined in the program policy and procedure.
  • Maintain scanning and complete additional filing projects, as requested, to support flow of documents through Outreach systems to timely completion consistent with Program operating protocols and audit requirements.
  • Ensure that an Outreach Supervisor is notified every time a member expresses interest in services so they can be reassigned to a Community Enrollment Specialist (CES) for enrollment services.

Qualifications

  • Minimum of one (1) year of job-related experience providing medical, mental health or substance abuse-focused services to individuals with chronic medical conditions and/or severe and persistent mental illness
  • High school diploma required, Associates degree preferred
  • Requires thorough knowledge of a specialized or technical field. May compile objective and measurable data and produce reports. May require operating knowledge of manufacturing or computer equipment, OR may require broad knowledge in clerical or service areas.
  • Excellent interpersonal, organizational, telephonic, writing and computer skills
  • Ability to engage clients and diffuse difficult situations
