    Program Overview:

    NADAP's Project ACE (Assessment*Case Management*Employment) is an intensive, employment-focused case management program that helps more than 700 New York City residents.  Staff conducts intensive client assessments and then designs and implements comprehensive, individualized service plans.  By addressing housing, substance abuse, mental conditions and legal barriers, Project ACE helps clients move toward economic independence and self-sufficiency.

    Responsibilities

    Responsibilities:

    The Clinical Case Manager provides comprehensive clinical case management services to individuals in need of substance use treatment and also behavioral health interventions and supports by assessing the client’s needs, developing individualized recovery plans (that include employability and rehabilitation) and coordinating services with treatment programs and community-based organizations.

    • Completes a case management assessment to identify client needs and barriers to self-sufficiency (medical, mental health, alcohol/substance abuse treatment, housing, legal, public benefits, vocational, educational, childcare, and other needs)
    • Manages multiple tasks to completion addressing client needs to facilitate self-sufficiency
    • Performs site visits to locations including the client's home, treatment programs, work or training assignment sites, employment site, or other community-based agencies
    • Utilizes multiple electronic database systems to monitor and address client problems related to mental health, medical, and substance abuse treatment
    • Evaluates quality, quantity, and effectiveness of health services. Assesses attendance, participation, and progress in a treatment program and/or work activity
    • Collaborates with internal and external partners in developing individualized service plans, conducting case conferences, and participating in clinical supervision
    • Collaborates with team members, including employment specialist, social workers and treatment providers to ensure that client is meeting goals and objectives of the individualized service plan

    Qualifications

    Qualifications:

    • Master’s degree in social work, psychology, rehabilitation counseling, or other related field is required
    • 3+ years of social service experience with at least one year experience in a clinical setting
    • Knowledge of substance abuse treatment modalities and New York City’s alcohol and substance abuse treatment system
    • Knowledge of the DSM-5, treatment of clinical disorders, and of community-based treatment modalities
    • Excellent verbal, written communication, problem solving, and organizational skills
    • Proficient in MS Office and using computers
    • Bilingual in Spanish preferred
