    Program Description:

    NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs. 


    Job Title: Intern Program Supervisor – New York, NY


    Job Summary:

    The Intern Program Supervisor (IPS) provides clinical and administrative supervision for a team of any combination of BSW and MSW Interns. The IPS works closely with his/her team to ensure consistent provision of high quality outreach, assessment, intervention, referral, linkage, monitoring and service planning for all persons served. IPS tracks programmatic data reports to ensure success service delivery and builds relationships with Health Home network medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for all clients.

    The IPS maintains and cultivates existing relationships with area BSW and/or MSW programs and Field Placement offices, and works with those programs to recruit and on-board appropriate BSW Interns.  IPS provides task and clinical supervision to the group of Interns, and completes evaluations as prescribed by the identified universities.


    • Ensures clients are provided effective care coordination services according to program requirements by mentoring and monitoring the Intern cohort, and intervening when necessary. Monitoring will be achieved by utilizing management tools, databases and multiple Electronic Health Records systems. Develops and implements action plans to improve effectiveness and efficiency of Interns, while ensuring a safe learning experience for each.  Meets with clients and conducts site visits as needed.
    • Provides clinical and administrative supervision to a team of any combination of BSW and MSW Interns.  Provides program orientation and training regarding department procedures and operations. Monitors Intern performance, leads individual and team clinical supervision meetings, assists Interns in problem solving to achieve client and program goals. Composes and delivers evaluations to appropriate universities.
    • Creates policies, procedures and workflows as needed, in conjunction with departmental management staff. Works with Systems Manager to create database reports as needed to monitor program requirements and ensure service and billing compliance.
    • Cultivates relationships with area MSW and BSW programs.  Ensures consistent and reliable communication.  Screens and selects appropriate MSW and/or BSW Interns for each term.  Provides appropriate and helpful feedback consistently to both the individual BSW/MSW Intern, and the corresponding University Program. 
    • Implements structured quality improvement (QI) activities. Reviews client records, including databases and Electronic Health Records to evaluate effectiveness of services. Contacts and meets with clients to follow-up on Interns' service delivery. Documents QI activities. Provides feedback and training to Interns based on QI outcomes.
    • Provides structured crisis intervention services as necessary to address client needs and resolve problems to promote client wellness. Interfaces with lead Health Homes, emergency services, Call Centers and other providers as needed.
    • Reviews assessments, services plans, contact notes and level of care determinations for all clients referred to program. Monitors caseload size and mix.
    • Assists with Health Home care coordination operations to meet the Program’s established goals. Proactively assists in the planning and implementation of policies and procedures. Provides program management with continuous feedback about operations and quality improvement reviews.
    • Collaborates with lead Health Homes and referral sources including clinical care providers and legal entities through participation in various stakeholder committees, meetings, and case conferences. Facilitates case planning and resolves participant service problems within the medical, behavioral health and social service systems, as necessary.
    • Performs other duties as assigned.


    • Master’s Degree in social services or behavioral health related field required 
    • Three (3) years of job-related experience supervising staff who deliver medical, mental health or substance abuse-focused services to individuals living with chronic medical and/or severe and persistent behavioral health needs
    • SIFI Certificaton 
    • LMSW or LCSW 
    • Excellent interpersonal, organizational, writing and computer skills
    • Working knowledge of medical and/or behavioral health care environments including diagnoses/assessment, clinical terminology, documentation standards and health information systems strongly preferred
    • Bilingual  a plus
