Program staff members link individuals to chemical dependency treatment and also reviews progress to assist them in recovery leading to self-sufficiency.   The AG conducts substance abuse assessments under the supervision of a qualified CASAC, determining clients’ appropriate leave of care and making referrals to treatment when indicated. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs.
