Position Overview:
Maintain on-going contact with CareerCompass clients that have been referred to CareerAdvance, Alternative Engagement (education, internship, community service) or other HRA Programs/services. Conduct regular and ongoing outreach calls to support enrollment and engagement in CareerCompass Program. Assist Internship and Placement Coordinator with tracking employed CareerCompass clients through the 30-day, 180-day and 365-day employment milestones. Identify any personal and social problems that may arise during employment or service placement and make appropriate referrals to assist the client. Meet with clients for any HRA scheduled re-assessment appointments.

NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs.

Program Overview:
SACAP (Substance Abuse Centralized Assessment Program) provides assessment and referral services to public assistance clients who may have substance abuse related barriers to work. Program staff members link individuals to chemical dependency treatment and also reviews progress to assist them in recovery leading to self-sufficiency.
The AG conducts substance abuse assessments under the supervision of a qualified CASAC, determining clients’ appropriate leave of care and making referrals to treatment when indicated. The Assessment Generalist (AG) also reviews Quarterly Progress Reports submitted by treatment programs to clinically assess if the client is meeting goals towards employment and self-sufficiency.
Position Summary
The AG conducts substance use assessments under the supervision of a qualified CASAC, determining clients' appropriate level of care and making referrals to treatment when indicated.
