    Program Overview:

    NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs.


    Job Title: Senior Outreach and Engagement Specialist


    Position Summary:
    The Senior Outreach and Engagement Specialist (SOES) provides written, face to face and telephonic outreach, engagement, enrollment, and orientation services for all referred members while embedded at an offsite location. The Senior Outreach and Engagement Specialist will work closely with community-based partners to identify, assess and enroll as many referred clients as possible. The SOES will act as a liaison between organizations by providing training and facilitating relationship development.


    • Provide intake and screening for all referred members. Complete all required enrollment/screening/assessment tools and consent forms mandated by the Health Home and/or NADAP policy & procedure. Maintain regular follow up on cases as needed.
    • Maintain positive rapport with all affiliated community based partners
    • Provide progressive and meaningful face to face outreach and orientation services for each referred member in order to facilitate Health Home screening, orientation, enrollment, engagement and retention. 
    • Maintain accurate case records in HHDB and assigned EHR systems
    • Provide training support and mentoring for new staff members, or those identified by a supervisor as persons in need of additional training.
    • Provide continuous feedback to management regarding perceived progress or regress of any staff.
    • Uphold all HIPAA regulations and confidentiality protocols to record each interaction with every client served
    • Utilize telephonic and electronic outreach, internet databases, EHR(s), NADAP databases, coordination with referring entities/providers, and ePaces searches to obtain valid contact, insurance, demographic and diagnostic information as needed to locate eligible participants and coordinate enrollment, follow up, and needed status updates for each client.
    • Performs other duties as assigned


    • High School Diploma required
    • Minimum (1) year of experience in serving populations living with chronic medical and/or mental health conditions
    • Bilingual Spanish/English
    • Excellent interpersonal, organizational, writing and computer skills
    • Must be able to travel via mass transit within New York City 75% of the time
