    Program Overview:

    NADAP's Project ACE (Assessment*Case Management*Employment) is an intensive, employment focused case management program that helps more than 700 New York City residents. Staff conducts intensive client assessments and then designs and implements comprehensive, individualized service plans. By addressing housing, substance abuse, medical and mental conditions and legal barriers, Project ACE helps clients move toward economic independence and self-sufficiency.

     

     

    Responsibilities

     

    The Case Manager Supervisor (CMS) provides clinical supervision and coordinates work activities for Case Managers. CMS provides crisis management and interventions with clients, ensures teamwork, and ensures consistent services in accordance with program policies and procedures.

    • Provides clinical and administrative supervision to case managers. Assists staff in diagnostic case analysis to identify and implement appropriate interventions.
    • Evaluates and documents staff performance with the ability to provide accurate and supportive feedback.
    • Conducts meetings to review performance and to motivate staff towards achieving weekly and monthly benchmarks.
    • Develops and fosters working relationships with behavioral health treatment providers and all other service providers as needed (e.g., criminal justice system, child welfare agencies, etc.). Participates in interdisciplinary meetings as needed.
    • Ensures milestones are met through organization and time management of all monthly outcomes.
    • Supervises and coordinates, and at times performs, site visits with Case Managers, including visits to client homes, work sites, HRA sites, vendor sites, and treatment programs.
    • Effectively collaborates with internal department heads and staff members to support program and policy development, resource development, and continuity of effective client services.
    • Identifies training needs and participates in staff development and delivery. Provides training in areas such as new policies, procedures, and case management issues.
    • Interviews and recommends applicants for case manager positions. Provides Project ACE with orientation and job-specific training for new hires and interns.
    • Compiles statistical information on clients and client services. Submits written reports to program administration. Participates in the development of Funding Source Reports
    • Reports directly to the Assistant Director and performs all other related job duties as needed or requested by supervision including: Q/A, Team Census Management, Management of Performance milestones, etc.

    Qualifications

    • Master’s Degree in social services or behavioral health-related field is required with at least 5 years of progressive experience in social services/case management services.
    • Minimum of three (3) years of staff supervision with at least two (2) included experience of behavioral health-focused services for individuals moving towards self-sufficiency.
    • Must have knowledge of OASAS, OMH, HRA, and Vocational Rehabilitation programming and services.
    • Excellent interpersonal, organizational, writing, statistical, and computer skills with the ability and opportunity to provide public speaking, data reports, and project management.
    • Working knowledge of medical and/or behavioral health care environments including diagnoses/assessment, clinical terminology, documentation standards, and electronic health information systems strongly preferred.
    • Spanish speaking preferred
