  • Overview

    Provides leadership and administrative support for Health Homes Care Coordination; oversees its development, care coordination, and administrative operations.

    Responsibilities

    • Manages Health Homes Care Coordination program to ensure goals are achieved. Ensures contract and regulatory compliance of operations.
    • Defines scope of work of the program. Oversees implementation of services. Advises staff on service delivery. Monitors operations and adjusts as necessary. Develops and implements new program services. Creates operating plans.
    • Interacts with external business associates to develop networking opportunities. Promotes NADAP’s business and ensures NADAP’s best interests and needs are met. Develops relationships with funding sources and negotiates contracts where appropriate.
    • Ensures programs and departments meet fiscal targets.  Participates in creating budgets and forecasts. Adjusts revenue and expenses accordingly. Develops, monitors and revises budgets in coordination with SVP. Participates in and prepares for budget-to-actual meetings. Oversees billing process for departments. Balances agency and programs needs. Identifies cost saving measures.
    • Participates and contributes to meetings including: Managers’, Corporate Compliance. Assists with development of policies and procedures. Contributes to agency strategic planning. Participates in lobbying activities.
    • Participates in agency development activities including development meetings, RFP identification and evaluation, proposal writing and review, marketing and public relations. Assists in identifying new business opportunities.
    • Recruits, trains and supervises staff. Provides feedback on performance and completes performance appraisals. Develops corrective action plans as necessary. Manages employee relations. Advises and ensures that supervisory staff appropriately manages staff performance.
    • Develops and oversees activities to ensure that quality services are delivered. Uses data to evaluate quality. 
    • Oversees the development of written policies and procedures for programs/departments to create manuals and other material documenting processes.
    • Oversees the functional development and maintenance of data collection systems e.g., database. Develops and generates reports and statistics to analyze operations.

    Qualifications

    Master's Degree required, preferably in a related field.

    At least seven years relevant experience , with at least three years in Health Homes/care coordination

    At least four years of supervisory experience. Able to prioritize and delegate responsibilities. 

    Excellent interpersonal skils. Able to build partnerships internal and external to the organization.

    Excellent analytical and problem solving skills.

    Strong working knowledge of Microsoft Office.
