    Program Overview:

    NADAP's CareerCompass, one component in HRA’s initiative to address the unique needs of improving employment and training outcomes in NYC, provides orientation, comprehensive and individual employability assessments and service coordination for adults 25 years of age and older.  As the first step in HRA’s programming, CareerCompass will identify and connect individuals to an education, training and/or employment track.


    Job Title:  Post Placement Coordinator


    Job Summary:

    The Post Placement Coordinator maintains on-going contact with clients that have been referred to Career Advance, Career Education Services or other HRA programs.  The Coordinator assists with tracking employed clients through employment milestones and identifies any personal and social problems and makes appropriate referrals.


      • Conduct regular outreach to program participants that have been referred into CareerAdvance, Alternative Engagement and other external HRA programs/services, in an effort to consistently track and document their progress on goals as outlined in their CareerCompass Employment Plan (EP) and Individual Service Plan (ISP).
      • Maintain on-going contact with those participants in an effort to identify any personal and social problems that may arise during employment and make appropriate referrals for the resolution. Identify barriers and issues that may result in participant being returned to CareerCompass for further evaluation.
      • Carefully review case notes, NYCWAY action inquiry, employment plan and individual service plan for all clients prior to initiating outreach.
      • Conduct regular outreach and service coordination with CareerAdvance providers, Alternative Engagement providers, and other HRA Programs/services staff to obtain accurate updates on client progress.
      • Conduct intake with all clients who report to orientation with Full Time Employment, obtain employment verification, enter FIA and track clients through the 30-day, 180-day and 365-day employment milestones.
      • Conduct re-assessment appointment with clients who are referred by HRA for 6-month reassessment and 1-year reassessment, update the Employment Plan and Individual Service Plan during these appointments
      • Provide provide case management, counseling and assist in the re-direction of alternative programming that might be a better fit for clients who are not progressing at their external engagements
      • Maintain and execute documentation and workflow through computer-based programs and technology that draws on basic software/computer skill set.
      • Attend and participate in meetings, staff development sessions and in-service training as assigned.
    • May be assigned other tasks and duties reasonably related to their job responsibilities.




    • Education and Experience: High School Diploma required with at least 3 years related work experience. Bachelor’s Degree in social services, vocational rehabilitation or related field preferred.
    • Must be able to work with online database systems.
    • Strong Internet research skills with excellent PC literacy [Excel, Outlook, Word].
    • Some experience working within a community-based organization or the government sector, directly serving an under-resourced population required.
    • Must be detail oriented, organized, ethical, responsible, & self-motivated
    • Experience working in employment services or in social service-related.
    • Bilingual in Spanish preferred.
