Overview

Founded in 1971, NADAP has been serving residents of New York City's metropolitan area for over 47 years. With various programs to assist New Yorkers in employment, health care, community support, and others, NADAP has been able to connect over 35,000 people to necessary services. In particular, our Health Home Care Coordination program has been working hard to connect New Yorkers to medical and behavioral services that improve their general health status and reduce the need for hospitalization and emergency care.

 

The Assistant Director of Health Homes Development reports to the Senior Director and oversees the Outreach, Quality and Data Management activities for the Health Home Care Coordination department. They directly supervise the Outreach Management Team, Quality Manager and Data Manager and work with the entire team to execute strategic plans to further develop the program, create efficient and actionable data systems and maintain consistent census growth.

Responsibilities

 

  • Increases enrolled Health Home Care Coordination membership through a variety of activities including soliciting referrals from communitybased partners and developing new initiatives with Health Homes and other agencies. Works to achieve the enrollments projected in the annual HHCC Operational Plan.
  • Provides oversight to Outreach, QA and Systems teams. Directly supervises Outreach Management Team, QA Manager and Data Manager, including monitoring caseloads, team productivity, and overall staff performance. Completes performance appraisals and manages employee relations issues. Collaborates with Care Coordination Managers to ensure that outreach operations complement care coordination policies and procedures.
  • Oversees relationships with community-based partners that provide referrals to NADAP. Monitors referral flow and works with community-based partners to adjust as necessary to produce sufficient referrals to meet program goals.
  • Leads the Outreach Development Supervisors in building new partnerships with community-based organizations to provide additional referral streams to NADAP. Follows up with new referral sources, and provides resources, data, and support to help establish lasting relationships.
  • Oversees recruitment and hiring of outreach , QA and systems staff. Evaluates staff training needs and coordinates staff trainings using internal and external resources as appropriate to ensure that staff has the skills necessary to meet expectations. Recommends revisions to the Senior Director when hiring or staffing structures could be adjusted to better meet department needs.
  • Oversee all aspects of HHCC’s internal and external data management systems and statistical reporting for department.
  • Collaborate with Senior Management to develop and implement new systems when needed. Collaborate with Senior Management and NADAP Database Team to identify areas where technology can increase efficiency for clinical and support staff; conceive, develop, and implement solutions. Ensure that HHDB and HH EHRs data systems are integrated into daily operations
  • Performs other duties as assigned. 

Qualifications

  • Master's Degree in Social Work, Mental Health Counseling, Public Administration, Public Health, or a related field
  • 3 years of experience in supervising staff in a program that delivers medical, mental health, or substance abuse focused services to individual. Preference given to individuals with experience overseeing supervisory level staff. 
  • Excellent written and verbal communication and organization skills
