The Peer Recovery Advocate’s primary function will be to support clients as they transition into employment.  This will be achieved by regular outreach to clients by phone, text, email, virtual meeting app, on-site visits, and traveling into the community to meet clients.  The Peer Recovery Advocate position requires a highly organized mentality to effective track a client’s progress through services and schedule client appointments as appropriate.  CRPA will be responsible for documenting all client interactions and client employment info in an electronic data management system.  The CRPA will be sensitive and proactive in supporting individuals who are maintaining recovery, including those still participating in treatment programs.  Additionally, a CRPA should have community resources available for clients who require additional supports related to treatment and other service needs.

Assists clients in determining their career pathway and education planning. Assists in the career decision making process to help clients identify their strengths, experiences, and vocational interests. Support clientsin making appropriate referrals based on assessment. Assist Internship and Placement Coordinator with conducting follow-ups. Identify any personal and social barriers that may arise during employment or service placement and make appropriate referrals to assist the client.

Position Summary The Training and Education Supervisor provides clinical and administrative supervision for a team of Social Work Interns. The Training and Education Supervisor works closely with their team to ensure consistent provision of high quality outreach, assessment, intervention, referral, linkage, monitoring and service planning for all persons served. Training and Education Supervisor tracks programmatic data reports to ensure success service delivery and builds relationships with Health Home network medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for all clients.   The Training and Education Supervisormaintains and cultivates existing relationships with area social work programs and Field Placement offices, and works with those programs to recruit and on-board appropriate Interns. Training and Education Supervisorprovides task and clinical supervision to the group of Interns, and completes evaluations as prescribed by the identified universities.   The Training and Education Supervisor works as a vital part of the Training and Education Team within the Health Homes Care Coordination Department.  They help to create, and modify training materials, and regularly facilitates teaching and training sessions for groups throughout the HHCC department.  The Training and Education Supervisor uses their clinical background, and technical skill, to assist in the clinical and skill development of the larger team.
