Program Overview:
Project ACE (Assessment*Case Management*Employment) is an intensive, employment-focused case management program that helps more than 700 New York City residents. We are dedicated to assisting clients identify their barriers to employment by completing a comprehensive assessment and by referring qualified clients for case management services. By addressing housing, substance abuse, medical/mental conditions and legal barriers, Project ACE helps clients move toward economic independence and self-sufficiency. That is our goal!

Program Overview:
NADAP's CareerCompass, one component in HRA’s initiative to address the unique needs of improving employment and training outcomes in NYC, provides orientation, comprehensive and individual employability assessments and service coordination for adults 25 years of age and older. As the first step in HRA’s programming, CareerCompass will identify and connect individuals to an education, training and/or employment track.
We are seeking an Administrative Assistant to provide administrative support to CareerCompass program and management, complete various projects to assist in the achievement of program goals and provide support for the front desk. Provide assistance in the following areas: scoring of assessments, time keeping, attendance tracking. This position reports to the Business Manager and work along the other Administrative Assistants and Receptionists.

NADAP’s Health Home Care Coordination program works in partnership with medical and behavioral health providers to align services that promote access to care and enhanced health outcomes for Medicaid recipients with a history or risk of over-utilizing medical and behavioral health services. Using an integrated medical-behavioral health approach, our team conducts face to face and telephonic outreach, provides assessment, intervention, referral, linkage, monitoring and service planning for individuals with complex medical conditions, severe mental illness, substance abuse and long-term care needs. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs.
Job Summary:
NADAP, Inc. is seeking a Care Coordinator for our Health Homes program to coordinate medical, mental health and substance abuse services for Medicaid recipients. Using an integrated medical and behavioral health home approach involving fieldwork and telephonic contact, our Health Home Care Coordination service conducts outreach, assessment and service planning to coordinate care for participants who have severe and persistent mental illness and/or chronic medical conditions. Care Coordination staff work closely with clinical service providers and deliver interventions to manage participants’ medical and behavioral health services. Care coordination helps participants access and effectively use clinical services to achieve better health care outcomes while containing costs.

The Orientation Facilitator provides interactive and motivational orientation to HRA CareerCompass participants including administering and scoring assessment tests.

NADAP is seeking a full-time Human Resource Assistant to support the HR Department and employees. S/he will ensure that day-to-day department operations run effectively and efficiently. S/he will be an integral part of the HR team and play an essential role within the department. The HR Assistant is the first point of contact for new hires and existing employees regarding benefits. S/he is responsible for all HR transactions.

The Nassau Employment Initiative (NEI), funded by the NY State Office of Temporary and Disability Assistance, is designed to provide outreach, case management, job preparation and job placement along with wrap-around services to individuals with HIV/AIDS in Nassau and Suffolk Counties. The program also provides post-placement supports and off site referrals to meet the needs of our participants.
Position Summary: Employment Specialist provides employment and career counseling, job preparation, job development, job referral and retention to Nassau Employment Initiative (NEI) participants.
