Position Summary

Career Services Coordinator (East Harlem) will provide comprehensive intake, assessment, service plan and coordination services for clients referred to CareerCompass. Assists clients in determining their career pathway and education planning. Assist Internship and Placement Coordinator with conducting follow-ups. The Assistant Director oversees the case management and workforce development teams to meet all program goals and milestones. He/she plans, develops, enhances and implements the program. The Assistant Director reports to the Director.

Job Summary:

The Outreach Coordinator(ORC) provides written, face to face and telephonic outreach, engagement, enrollment, and orientation services for all referred members while embedded at an offsite location. The Outreach Coordinator will work closely with community-based partners to identify, assess and enroll as many referred clients as possible. The ORC will act as a liaison between organizations by providing training and facilitating relationship development.

The Database Manager will liaise with department leaders, the Vice President, and the Credible Business Partner to resolve issues. The objective of the position is to ensure that all NADAP departments utilize Credible effectively and meet the data tracking and reporting requirements of the agency.

Founded in 1971, NADAP has been serving residents of New York City's metropolitan area for over 46 years. With various programs to assist New Yorkers in employment, health care, community support, and others, NADAP has been able to connect over 35,000 people to necessary services. In particular, our Health Home Care Coordination program has been working hard to connect New Yorkers to medical and behavioral services that improve their general health status and reduce the need for hospitalization and emergency care.

 

The Senior Outreach Development Supervisor provides oversight to community based partnerships that provide referrals to NADAP. The Senior Outreach Development Supervisor is also responsible for providing clinical and administrative supervision for a specialized outreach team. Senior Outreach Development Supervisors are responsible for sourcing and engaging with potential community services with the goal of developing new partnerships and developing a strong community presence to increase referrals to the Health Home Care Coordination program.
