The goal of the program is to educate and assist individuals and small businesses to enroll in the Health Insurance Marketplace including Medicaid, Child Health Plus, the Essential Plan, and Qualified Health Plans. Navigators will work with participants to enroll them online into the NY State Health Insurance Marketplace.

The Peer Recovery Advocate (CRPA) work business hours Monday-Friday. Competitive salary and excellent health package including tuition reimbursement, and loan forgiveness program.   Program Overview: SUCAP (Substance Use Centralized Assessment Program) provides assessment and referral services to public assistance clients who may have substance abuse related barriers to work. Program staff members link individuals to chemical dependency treatment and also reviews progress to assist them in recovery leading to self-sufficiency.   The Peer Recovery Advocate (CRPA) will provide strengths-based guidance and substance use recovery support to clients to motivate and assist them with making a decision to engage in treatment. The Peer Recovery Advocate will help clients resolve barriers to achieving self-sufficiency. The Peer Recovery Advocate will work with clients at 16th Street and at HASA sites The CRPA will also provide training and dispensation of Narcan kits to clients at those sites, as well as conduct Narcan Fairs for HRA clients.

The Assistant Manager will assist New York residents with enrolling into the NY State of Health Insurance Marketplace and educate the public on the Marketplace. He or she will also assist in training new Navigators and compiling monthly reports for partner organizations. Care Coordinators work closely with networks of clinical service providers to manage identified needs, stabilize participants and reduce health care costs.      Job Summary:                                                                                    NADAP, Inc. is seeking a Care Coordinator for our Health Homes program to coordinate medical, mental health and substance abuse services for Medicaid recipients.  Using an integrated medical and behavioral health home approach involving fieldwork and telephonic contact, our Health Home Care Coordination service conducts outreach, assessment and service planning to coordinate care for participants who have severe and persistent mental illness and/or chronic medical conditions.  Care Coordination staff work closely with clinical service providers and deliver interventions to manage participants’ medical and behavioral health services. Care coordination helps participants access and effectively use clinical services to achieve better health care outcomes while containing costs.
